About Your Visit and Office Forms
Welcome to the office! Please complete the forms provided on my web site prior to your visit. If you would like us to bill your insurance company, don’t forget your insurance card and co-pay which must be presented at the time of service. For those who do not have insurance coverage, it is expected that charges be paid in full at the time of each visit. If this is your first visit to this office, please arrive 15 minutes before your scheduled appointment.
The doctor makes every attempt to see you at the time of your appointment. In respect to you and the other patients that have appointments, please arrive on time for your appointment. If you have had any audiograms, x-rays or laboratory studies relating to your visit, please bring those with you.
Please do not bring children unless they are scheduled for an appointment.
We serve communities throughout the county of San Diego as well as Orange and Riverside counties. We are closely located to Del Mar, Rancho Santa Fe, Carmel Valley, La Jolla, Encinitas, Carlsbad, Oceanside and San Marcos. We can be easily reached from inland communities such as Poway, Rancho Bernardo, Escondido and Rancho Penasquitos. We are also just a short drive from downtown San Diego and surrounding communities.
We are affiliated with Scripps Memorial La Jolla and Encinitas Hospitals and have privileges at free-standing surgical centers including University Ambulatory Surgery Center.

Billing and Insurance
If you would like us to bill your insurance company, don’t forget your insurance card and co-pay which must be presented at the time of service. For those who do not have insurance coverage, it is expected that charges be paid in full at the time of each visit.
We accept most PPO insurances, Medicare and cash patients. We now have an affiliation with Scripps Coastal and Scrips Physicians Medical Group. Dr. Cynthia Davis is a Qualified Medical Examiner for Worker’s Compensation Medical-Legal Cases.
We accept most insurances including:

- Medicare (not MediCal)

- Healthnet Covered California*
- Multiplan
* Only certain plans are covered.
We also see patients with referrals, who have HMO health plans affiliated with Scripps Coastal Medical Center and Scripps Physicians Medical Group such as:

- SCAN
We also care for many patients that do not have health insurance or those that are being seen out of their insurance networks. For uninsured patients, we can also make special payment arrangements.
