Dr. Salgado and Dr. Goldsztein received the San Diego County Medical Society Health Heroes "Champion" Award

Dr. Salgado and Dr. Goldsztein received the San Diego County Medical Society Health Heroes "Champion" Award on Monday, April 18, 2016 at the Lomas Santa Fe country Club. They were awarded the honor for providing access to healthcare for disadvantaged San Diegans.   They worked with Project Access San Diego, performing intricate surgical procedures to treat individuals with massive goiter (enlarged thyroid glands) and thyroid cancers.  Doctor's Salgado and Goldsztein performed the surgical procedures and postoperative care without charge, and Scripps Hospital provided the operative and hospitalization services requires for their procedures.

2nd World Head and Neck Cancer Day - July 27th 2016

This day is dedicated to help disseminate knowledge related to prevention, early detection, affordable care, evidence based management, rehabilitation for those individuals with head and neck cancers.  We invite individuals who are concerned about being at risk for head and neck cancers to make an appointments to be screened by one of our Practice's head and neck surgeons.

Risk factors for head and neck cancers include tobacco and alcohol use and exposure to HPV 16 (human papilloma virus).  Less common causes included a diet high in animal fats and low in fruits, exposure to certain chemicals such as hardwood dust, leather dust, formaldehyde, and having leukoplakia and erythroplakia (white or red patches) in the mouth.

Signs and symptoms of head and neck cancers include chronic sore throat or hoarseness, difficulty swallowing, having a lump in the neck, oral ulcer or sore' persistent red or white patch in the mouth.  Other symptoms include pain or difficulty chewing, swallowing or moving the jaw or tongue, unexplained weight loss, blood in the saliva or phlegm, which appears in the mouth mucus and  foul mouth odor not explained by hygiene.

Examinations for head and neck cancer include a thorough examination of the head and neck region and flexible laryngoscopy,  which is a flexible fiberoptic examination scope that allows the doctor to further evaluate the back of the nose, throat and larynx (voice box).  The procedure is performed in the office using topical local anesthesia and causes minimal discomfort.  If indicated, further tests are ordered, including imaging studies, biopsies or laboratory studies.

We are pleased to announce and welcome Dr. Hernan Goldsztein to the practice!

World Voice Day is April 16, 2015world voice day

Despite the marked increase in use of electronic communication, the voice is still the primary means of human communication. Life is enriched by telling someone you love them, singing a beautiful song or alerting someone of imminent danger. Voice communication is more efficient and more effective in emitting emotion. Social interaction is enhanced with vocal communication.

We care about your voice. Otolaryngologists (Ear, Nose and Throat doctors) specialize in the diagnosis and care of voice disorders. Whether or not you have laryngitis (acute inflammation), nodules, polyps or in the worst case scenario, cancer, we can evaluate and offer treatment for your voice condition. During a regular office visit, we can conduct an examination of the larynx (voice box). With the use of a flexible instrument (laryngoscope), we can determine the nature and cause of your voice disorder. At Pacific ENT Medical Group, we are one of the few offices in San Diego with a high definition laryngoscope and stroboscopy that allows us to better diagnose more subtle voice problems.

The U.S. Food and Drug Administration (FDA)  recently approved  3 sublingual tablets for the management of pollen-induced allergies. These new tablets offers new therapy options for those who suffer from nasal and upper respiratory allergies.  One sublingual tablet can be placed  under the tongue, once each day, and left there until it dissolves. Shots and Drops remain a viable option for managing specific allergies, and Tablets will now represent another method of allergy management. Similar in composition to Shots and Drops, the Tablets are formulated to change one's immune system's response. This gives patients another option for at-home management of their allergies.

Sublingual tablets are only available by prescription, and must be started before the onset of the allergy season.  Prior to starting therapy, either skin testing or blood studies must be performed, to confirm that an individual is allergic to the specific allergen which is contained in the tablets.
The 3 sublingual tablets available are:

RAGWITEK (TM) by Merck is a sublingual immunotherapy tablet approved for the treatment of short ragweed pollen-induced allergic rhinitis (nasal symptoms)  with or without conjunctivitis (eye symptoms)

ORALAIR (R)   by Greer , is indicated for the treatment of grass pollen-induced allergic rhinitis with or without conjunctivitis for any of the five grass species contained in the product ( Sweet Vernal, Orchard, Perennial Rye, Timothy and Kentucky Blue Grass).  This one is approved for ages 10-65.

GRASTEK (R) by Merck also is indicated for the treatment of grass pollen-induced allergic rhinitis with or without conjunctivitis for Timothy Grass or cross-reactive grass pollens that include Meadow fescue, Rye, Kentucky blue, Orchard and Red top.

Allergy immunotherapy is generally well-tolerated but may be associated with mild reactions such as itching of the mouth, lips, tongue or throat.  Rarely, there may be severe, life threatening  allergic reactions, such as trouble breathing, throat tightness or swelling, dizziness, fainting or trouble swallowing or speaking.

At Pacific ENT Medical Group, we offer comprehensive allergy testing and treatment including intradermal skin testing, allergy shots, sublingual drops,  and most recently, tablet therapy.  You may schedule an appointment to discuss your treatment options.

World Voice Day – April 16.
Theme : Educate Your Voice

Voice is produced from the larynx which is the sound box behind the adam’s apple in the neck. Vocal folds also called vocal cords, within the larynx vibrate to produce sound. The sound is shaped by other structures such as the throat, mouth and nose, to produce speech and song.

Otolaryngologists (ear, nose, and throat physicians) specialize in the examination and diagnoses of voice disorders. If your voice is hoarse, raspy or there is a change in pitch, you likely have a disorder of your larynx. Disorders of the voice are diagnosed by using a fiberoptic laryngoscope, an instrument that is passed through the nose or mouth to visualize the vocal cords.

Generally, if symptoms resolve after a few days and do not recur regularly, there is probably nothing to worry about. Indications for seeing an otolaryngologist include abrupt onset and severe change in voice that may be due to vocal cord hemorrhage (bleeding) or voice problems that lasts longer than two weeks, especially in someone who smokes.

Things that you can do to help maintain the health of your voice:

  • Don’t smoke
  • Keep well hydrated. Water is best.
  • Don’t scream or shout. Consider using a microphone if you need to project your voice for extended periods of time
  • Rest your voice if you develop hoarseness or laryngitis
  • Get evaluated by an otolaryngologist if you have persistent hoarseness.

Pacific ENT Medical Group focuses on voice disorders and is equipped with the state of the art diagnostic and treatment equipment including laryngeal videostroboscopy, microlaryngoscopy, laryngeal Botox therapy, laryngeal microsurgery, framework surgery (for vocal paralysis or atrophy), and awake laser therapy.

We are now able to perform diagnostic ultrasound examinations and ultrasound guided biopsies of the head and neck region.

Thyroid and parathyroid glands and lesions, enlarged lymph nodes and other head and neck masses can be evaluated by ultrasound. The procedure is highly accurate, noninvasive, and does not expose one to radiation. Ultrasonography is helpful in the diagnosis and differential diagnosis of any pathologic masses in the entire neck and prior to any surgical procedure. The procedure can be performed rapidly in the office reducing any delays in evaluating lesions or swelling in the head and neck region. There is no discomfort associated with the examination.

Dr. Davis has received certification of completing the American College of surgeons Ultrasound Course for Surgeons and Ultrasound Examination of the Thyroid and Parathyroid Glands.

Balloon Sinuplasty Update January 28, 2013

Balloon sinuplasty is one of the procedures available to treat chronic sinus disease. Dr. Davis was the first surgeon in San Diego to perform this procedure following its introduction in 2005.

The procedure has been successful in treating acute and chronic sinusitis of the maxillary, frontal and sphenoid sinuses. It has also been used to treat barotrauma of the sinuses (inability to equalize the pressure in the sinuses with diving and flying, resulting in pain in the involved sinus). Until recently, it was only performed in the operating room, primarily because insurance did not cover this procedure in the office setting. Last year, Medicare and most medical insurance carriers, began covering this procedure performed in the office. For appropriate patients, this procedure can be performed under local anesthesia obviating the need to undergo a general anesthesia.

Four recently published studies support the safety and effectively in achieving improvement in both sinus symptoms and the appearance of sinus CT scans in individuals treated with office-based balloon dilation. Those studies revealed 93.2% successful dilation of the sinuses in the office setting. The majority of patients were able to return to normal activities within 2 days of the procedure.

Human Papillomavirus in Orophryngeal Cancers

Many cancers that develop in the head and neck region are related to the use of tobacco products. Over the past several years, a virus human papilloma virus (HPV), has been implicated in the development of cancers involving the tonsils and tongue. In fact, 70% of these cancers are now associated with the virus. Those individuals that develop cancer are more often younger, male and non-tobacco users. High-risk sexual activity is the major risk factor for developing the disease. Symptoms of cancer may be a chronic sore throat or a neck mass.

Fortunately, the prognosis for those with HPV related oral cancer is better than those associated with tobacco. Of course, what is most important is that the cancer is diagnosed and treated as early as possible. A chronic sore throat or unexplained neck mass should be evaluated. Examination in the office and imaging studies are used to evaluate these tumors.

The first balloon catheter for sinus surgery developed by Acclarent, became available for clinical use 3 years ago. Dr Davis was one of the first surgeons in California, and the first in San Diego, to use this surgical tool. Because of her extensive experience with this new approach to sinus surgery, she has instructed physicians across the country on the use of the balloon sinusplasty catheter.

The balloon catheter allows for quicker recovery and less scar tissue formation because the sinus cavities are opened by expanding the bone and tissue with a balloon (like angioplasty) rather than cutting.

This year, another balloon catheter has been developed by Entellus to treat chronic maxillary and ethmoid sinusitis. This balloon provides a more direct approach to the opening of the sinus by going under the lip, rather than through the nose. It does not require the use of X-ray during the procedure, and may be performed with twilight or general anesthesia. Dr Davis was also the first to perform this procedure in San Diego.

Both of these new approaches offer a less invasive approach to the treatment of chronic sinus infections.

Update on Balloon Catheter Sinusotomy

In July 2007, a study was published in the Otolaryngology Head and Neck Surgery Journal that reported on the results and safety of Sinus Surgery using the Balloon Catheter. This study reviewed the results of 115 patients at 9 clinical centers over a 6 month period. The Balloon Catheter technology was found to be safe and effective. Patients experienced improvement of their symptoms following their surgery. In addition, the success rate was 97% which compares favorable to traditional endoscopic surgery that has a success rate of 75-90%.
