Welcome to the office! Please complete the forms provided on my web site prior to your visit. If you would like us to bill your insurance company, don’t forget your insurance card and co-pay which must be presented at the time of service. For those who do not have insurance coverage, it is expected that charges be paid in full at the time of each visit. If this is your first visit to this office, please arrive 15 minutes before your scheduled appointment.

The doctor makes every attempt to see you at the time of your appointment. In respect to you and the other patients that have appointments, please arrive on time for your appointment If you have had any audiograms, x-rays or laboratory studies relating to your visit, please bring those with you.

Please do not bring children unless they are scheduled for an appointment.

Map & Directions

We have provided directions to our office and our office building sign which can help guide you to our office. If you have any further questions regarding directions or visit preparation please contact us directly. 858-755-9343

From I-5 heading North or South:

Exit Lomas Santa Fe Drive. Go west onto Lomas Santa Fe Drive from the off-ramp. Go approximately ½ mile, past the elementary school on your right and Earl Warren Middle School on your left. The office building is located on your right (North) shortly after you have passed Glencrest Road. The building is one-story, white with blue trim. The office is in the one-story building, and is in the East (closer to the freeway) part of the office complex. There is ample parking by the office.

From Hwy 101 heading North or South:

Travel to Lomas Santa Fe Drive. Go east, away from the ocean. Go up the hill. Just past the fire station on your left is the medical office complex. It is white with blue trim. The office is in the one-story building in the East corner of the complex.

What should you bring to your appointment at Pacific ENT Medical Group, Inc

We want to answer as many of your questions at your visit.

Please prepare for this by bringing:

• Health Insurance Card/Information
• A friend or family member to take notes/help with questions
• A list of all medications you are taking
• A list of your questions /concerns
• A copy of your XRAYS / CT scans (not the reports - get the actual films)
• Copies of records from doctors related to your sinuses/nose/allergies/lungs.
• Completed FORMS Provided

Billing & Insurance Information

We accept most PPO insurances, Medicare and cash patients. We do not have any HMO affiliations. Dr. Cynthia Davis is a Qualified Medical Examiner for Worker’s Compensation Cases.
